Form of provision of result of rendering the state service: paper.";

"9. The list of the documents and materials necessary for rendering the state service in case of the address of uslugopoluchatel (or his representative by proxy) in State commission:
1) the statement on testing of selection achievement for economic usefulness, in duplicate, in form, according to appendix to this standard of the state service;
2) when carrying out testing and assessment of grades of agricultural plants on economic usefulness according to uslugopoluchatel - documentation confirming positive results of testing of the grade of authorized bodies of other states which is carried out in the conditions similar in edaphic-climatic parameters to conditions of the administrative and territorial unit recommended for use;
