In pursuance of Item provisions 13, last paragraph; Item 38, subitem 5); Item 52, subitem 2); Item 52, subitem 3); Item 92, subitem 5); Item 100, the subitem 3, of Regulations on procedure for distribution of means of National fund of development of the agricultural industry and the rural zone approved by the Order of the Government No. 455 of June 21, 2017 (further - the Provision), and for the purpose of creation of uniform conditions for ensuring effective administration with the financial resources intended for support of agricultural producers, I ORDER:

1. Approve:

1) the Standard statement for provision of financial support for each measure separately, according to appendix No. 1;

2) the Declaration under own responsibility of the agricultural producer, according to appendix No. 2;

3) the Business plan at least for 5 years, according to appendix No. 3;

4) the Standard act of pumping of water with small irrigation, according to appendix No. 4;

5) the Standard act of pumping of water on centralized systems, according to appendix No. 5;

6) Model of the certificate issued by financial institutions in the direction of use of the credit and the amount of paid interests, according to appendix No. 6;

7) Model of the list of payment orders, according to appendix No. 7;

8) The declaration of the agroproducer on ownership of combines, agricultural tractors and agricultural aggregates, according to appendix No. 8;

9) the Technical leaf, according to appendix No. 9.

2. Assign to interventions and payments Agency in agricultural industry ensuring publication of this order, including in the Official monitor of the Republic of Moldova, and placement on the official site of the Agency on interventions and payments in agricultural industry.
