1. Approve the attached Description of forms of the certificate on state registration of the legal entity, the certificate on state registration of physical person entrepreneur and the extract from the Unified State Register of Legal Entities and physical persons entrepreneurs.

2. To department of registration and licensing (Efremov O. V.):

in five-day time to provide provision of this order on state registration in the Ministry of Justice of Ukraine;

provide the organization of the order of production of forms of the certificate on state registration of the legal entity, forms of the certificate on state registration of physical person entrepreneur and forms of the extract from the Unified State Register of Legal Entities and physical persons entrepreneurs.

3. This order becomes effective along with entry into force of the Law of Ukraine "About state registration of legal entities and physical persons entrepreneurs".

4. To impose control of execution of this order on the vice-chairman Tretyakov S. I.
