According to part one of article 5 of the Law of Ukraine "About licensing of certain types of economic activity" and resolutions of the Cabinet of Ministers of Ukraine of November 20, 2000 N1719 "About Introduction of the License of Single Sample for Certain Types of Economic Activity"

I ORDER:

1. Approve the Instruction about procedure for filling of the form of the license of single sample it (is applied).

2. - to the head of department of licensing of types of economic activity of Department of registration and licensing (Efremov O. V.) to submit to the deputy director of Department of registration and licensing the Instruction about procedure for filling of the form of the license of single sample to five-day time on state registration in the Ministry of Justice of Ukraine.

3. To the head of department cases (Mintsevich E. M.) in ten-day time after state registration of the order "About Approval of the Instruction about Procedure for Filling of the Form of the License of Single Sample" to provide replication it and posylaniye to bodies of licensing.

4. To impose control of execution of this order on the vice-chairman Tretyakov S. I.
