Make changes to the unified forms of the statements which are drawn up by results of holding planned (unplanned) actions of the state supervision (control) by the Ministry of Internal Affairs, the Ministries of Internal Affairs of Ukraine approved by the order of February 4, 2016 No. 85, registered on February 25, 2016 for No. 294/28424, having been reworded as follows the Ministry of Justice of Ukraine:
