1. Bring in the order of the Minister of health and social development of the Republic of Kazakhstan of January 30, 2015 No. 44 "About approval of Rules of conducting medico-social examination" (it is registered in the Register of state registration of regulatory legal acts at No. 10589, it is published in information system of law of Ad_let on April 15, 2015) the following changes:

"1. These rules of conducting medico-social examination (further - Rules) determine procedure for need of osvidetelstvuyemy person for measures of social protection, on the basis of assessment of the restrictions of life activity caused by permanent disorder of functions of organism.";

to state part one of Item 2 in the following edition:

"2. Medico-social examination (further - MSE) is carried out by territorial subdivisions of Committee of work, social protection and migration of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan (further - territorial subdivisions).";

"Chapter 3. Procedure for survey (re-examination)";

state Items 8 and 9 in the following edition:

"8. MSE of the osvidetelstvuyemy (re-examined) person is carried out in the VKK direction of the medical organization:

1) at the permanent address (registration);

2) in the location of departments of MSE and (or) departments of methodology and control MSE of the respective region;

3) in the location on treatment in the organizations of health care giving stationary help;

4) in the location of the organizations of health care giving out-patient and polyclinic help;

5) in organizations of criminal executive system and in pre-trial detention centers.

9. Survey (re-examination) is carried out according to the statement on conducting medico-social examination in form according to appendix 1 to these rules with submission of the following documents:

1) the direction for medico-social examination in the form 088/at approved by the order of the acting minister of health care of the Republic of Kazakhstan of November 23, 2010 No. 907 "About approval of forms of primary medical documentation of the organizations of health care" (it is registered in the Register of state registration of regulatory legal acts at No. 6697) (further - form 088/at), no later than one month from the date of the direction specified in it;

2) the copy of medical part of the individual program of rehabilitation of the patient (disabled person) (further - medical part of the International Party of Russia), in case of its development by the medical organization for form according to appendix 2 to these rules;

3) the copy of the identity document of osvidetelstvuyemy person and the original for reconciliation;

4) the document confirming registration on the permanent residence (the address reference or the reference of the rural akim). The reference (in any form) confirming the fact of content of person in organization of criminal executive system or the pre-trial detention center;

5) the medical record of the ambulatory patient for the analysis of dynamics of disease. In case of availability of the copy of statements from the clinical record, the conclusions of specialists and results of inspections;

6) the copy of the reference (coupon) of attachment to the medical organization - in case of attachment to the PHC organization out of the place of permanent residence (registration), according to the Rules of rendering primary health care and Rules of attachment of citizens to the organizations of primary health care approved by the order of the Minister of health and social development of the Republic of Kazakhstan of April 28, 2015 No. 281 (it is registered in the Register of state registration of regulatory legal acts at No. 11268);

7) the leaf (certificate) of temporary disability - is provided by the working person, for introduction of data on the expert opinion;

8) the copy of the document confirming labor activity and (or) data on nature and working conditions on production (it is filled with the employer) in form according to appendix 3 to these rules - is provided (in the presence) in case of primary survey-faced working-age, provision in cases of industrial injuries or occupational diseases is obligatory;
