1. Approve Requirements and safety conditions (licensed conditions) of implementation of activities for use of sources of ionizing radiation and the Requirement to the report on the analysis of safety of implementation of activities for use of sources of ionizing radiation which are applied.

2. (V. Golubev) to provide to management of regulation of safety of radiation technologies provision of this order in five-day time on state registration in the Ministry of Justice of Ukraine.

3. (V. Golubev) to provide to management of regulation of safety of radiation technologies bringing to permission of the interested organizations of information on entry into force of this order and provision of explanations concerning its application.

4. To the state company "The State Scientific and Technological Center on Nuclear and Radiation Safety" (V. Vasilchenko) in a month after state registration to ensure replication of Requirements and safety of conditions (licensed conditions) of implementation of activities for use of sources of ionizing radiation and Requirements to the report on the analysis of safety of implementation of activities for use of sources of ionizing radiation.

5. To impose control of execution of the order on the First Deputy Chairman O. Mikolaychuk.
