About introduction of amendments to the Order of the Main state tax authorities No. 1804 of December 30, 2014

1. In the Order GGNI No. 1804 of 30.12.2014 about approval of the Single tax report / (Declaration) (the UNIF14) Form (Monitorul Oficial al Republicii Moldova, 2015, No. 11-21, Art. 115) to make the following changes and additions:

- the table No. 2 with the constituting appendix;

- table No. 5 "Fees of compulsory medical insurance".
