About change of the Order No. 1164 of the Main state tax authorities of December 25, 2012 about approval of the declaration form on the VAT and procedure for filling of the declaration on the VAT

1. Change the Order No. 1164 of the Main state tax authorities of 25.10.2012 about approval of the declaration form on the VAT and procedure for filling of the declaration on the VAT, as follows:

1) In the order:

a) in item 4 of the order the text of GP "Fiscservinform" to replace with the text Public institution "Center of information technologies in finance";

b) in Item 6 to "Management of methodology of administration" to replace the text with the text "Head department on methodology of taxes and fees";

2) In appendix No. 1 "The declaration form on the VAT"

a) to add the name of the column 7 "Import of Services" with the text "/the Return taxation";

b) Add with the columns 7.1 "Including" and 8.1;

c) In column 20 to "import of services" to add the text with the text "/to the return taxation".

3) In appendix No. 2 "Procedure for filling of the declaration on the VAT":
