1. Make changes to the form of the Tax declaration of collection for parking lots of vehicles approved by the order of the Ministry of Finance of Ukraine of July 9, 2015 No. 636, registered in the Ministry of Justice of Ukraine on July 29, 2015 for No. 912/27357 (with changes), having been reworded as follows it which is applied.

2. To provide to department of tax policy and supervision of fiscal bodies in the procedure established by the legislation:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

3. This order becomes effective from the date of its official publication.

4. To impose control of execution of this order on the Deputy Minister of Finance of Ukraine of Hodakovsky P. V. and the Chairman of the State Tax Administration of Ukraine Verlanova S.A.
