1.1. the notification on intention to perform microfinancial activities (further - the notification) the legal entities specified in part two of Item 3 of the Presidential decree of the Republic of Belarus of October 23, 2019 to No. 394 (further - legal entities), goes to National Bank on paper (registered mail) signed by the head of the legal entity (authorized person) or by means of system of interdepartmental electronic document management of state bodies in the form of the electronic document or the electronic copy of the paper document with use of the digital signature of the head of the legal entity (authorized person) (in case of its direction the authorized person - with application of a copy of the document confirming appropriate authority);

1.2. the notification contains the following data:

full name and location of the legal entity;

date of state registration and registration number in the Single state register of legal entities and individual entrepreneurs, the name of the body which performed state registration;

position, surname, own name, middle name (if that is available) the head of the legal entity;
