About change of resolutions of Council of Ministers of the Republic of Belarus of February 17, 2012 No. 156 and of August 11, 2012 No. 745

1. Make changes to the following resolutions of Council of Ministers of the Republic of Belarus:
1.1. to add the column "The List of the Documents and (or) Data Represented by Interested Persons to Authorized Body for Implementation of Ministerial Procedure" of Item 9.16 of the single list of the ministerial procedures performed by state bodies and other organizations concerning legal entities and individual entrepreneurs, the Council of Ministers of the Republic of Belarus approved by the resolution of February 17, 2012 No. 156, with the paragraph of the following content:
"the copy of the service record (the statement from the service record) the certified employee of public association of consumers certified in accordance with the established procedure";
1.2. in the resolution of Council of Ministers of the Republic of Belarus of August 11, 2012 No. 745 "About approval of the Regulations on procedure for carrying out employee assessment of public association of consumers and introduction of amendments and changes in some resolutions of Council of Ministers of the Republic of Belarus":
state the name in the following edition:

state preamble in the following edition:
