1. Make to the section I forms of the Declaration on property condition of the debtor in the case of the insolvency approved by the order of the Ministry of Justice of Ukraine of August 21, 2019 No. 2627/5, registered in the Ministry of Justice of Ukraine on August 23, 2019 for No. 964/33935, the following changes:

The name of the column second to state 2) in Item 7 in the following edition:

"Surname, name, middle name, birth date, nationality".

2. To management of justice and punitive justice (Oleynik A.) submit this order on state registration according to the Presidential decree of Ukraine of October 03, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".

3. This order becomes effective from the date of its official publication.

4. To impose control of execution of this order on the deputy minister concerning the European integration Kolomiyets V.
