4. Document forms about the birth of the child, the act of leaving of the child, and also the document form issued by medical institution, confirming age and sex of the found (thrown) child, affirm the Ministry of Health and the Medical Industry of Turkmenistan.

The document form about detection of the child issued by law-enforcement body affirms the Ministry of Internal Affairs of Turkmenistan, and issued by guardianship and custody body - the Ministry of Education of Turkmenistan.
