1. Approve provision Form to the central executive body realizing the state tax policy, information on the granted licenses to use radio-frequency resource of Ukraine concerning each payer of rent payment for use of radio-frequency resource which calculates the amount of rent payment according to line item of the 11th Item 254.4 of article 254 of the Tax Code of Ukraine which is applied.

2. In accordance with the established procedure to provide to department of licensing this decision on state registration to the Ministry of Justice of Ukraine.

3. This decision becomes effective from the date of its official publication.
