In case of detection in information and telecommunication networks, including in Internet network, unreliable information which discredits honor and advantage of the citizen (physical person) or hurts its reputation and it is connected with accusation of the citizen (physical person) in crime execution, such citizen (physical person) (further in this Article - the applicant) has the right to send to the prosecutor of the subject of the Russian Federation according to the procedure, established by the Prosecutor General of the Russian Federation, the statement for taking measures to removal of the specified information and to restriction of access to the information resources distributing the specified information in case of its not removal (further - the statement).

2. The statement shall contain:

1) surname, name, middle name, passport data of the applicant, contact information (phone numbers and (or) fax, e-mail address, postal address);

2) domain name of the website in Internet networks, its network address, the index of the page of the website in the Internet networks allowing to identify information specified regarding 1 this Article;

3) motivated reasons for unauthenticity of the placed information.
