1. The subitem 6 of Item 1 of the order of the Ministry of Justice of Ukraine of November 18, 2016 No. 3268/5 "About approval of forms of statements in the field of state registration of legal entities, physical persons entrepreneurs and public forming", registered in the Ministry of Justice of Ukraine on November 18, 2016 for No. 1500/29630, to state in the following edition:
"6) Statement for confirmation of information about the final beneficial owner of the legal entity (form 6);".
2. Form 2 "The statement for state registration of the legal entity (except public forming and authorities)", form 4 "The statement for state registration of the legal entity - public forming" and form 6 "Statement for confirmation of information about the final beneficial owner of the legal entity", the Ministries of Justice of Ukraine approved by the order of November 18, 2016 No. 3268/5, registered on November 18, 2016 for No. 1500/29630, to be reworded as follows the Ministry of Justice of Ukraine which are applied.
3. To department of private law (Ferens E.) submit this order on state registration according to the Presidential decree of Ukraine of October 3, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".
4. To the technical administrator of the Unified State Register of Legal Entities, physical persons entrepreneurs and public forming to take the measures necessary for implementation of this order.
