List of documents necessary for rendering the state service

State corporation:

1) the statement on putting down of apostille in form according to appendix 2 to Rules;

2) the identity certificate, or the electronic document from service of digital documents (for identification);

3) the document which is subject to apostilization (documents are accepted for putting down of apostille both in originals, and in notarially attested copies);

4) the document confirming payment in the budget of the state fee;

5) notarially certified power of attorney, in case of the address of the representative of uslugopoluchatel (physical person);

the power of attorney on behalf of the legal entity issued signed by his head or the other person authorized on it by its constituent documents and under seal this organization in case of the address of the representative of the legal entity;

The powers of attorney issued or certified by competent organization of foreign state or specially on that the authorized person, within its competence and in the established form sealed official foreign state, are adopted only after passing procedures of the special certificate (legalization or apostilization) (for identification with the copy);

6) the specimen signature and impress of a seal of the issued documents other state bodies.

In case of the address through the portal:

1) the electronic statement certified by the EDS of uslugopoluchatel or certified by the one-time password, in case of registration and connection of subscriber number of the uslugopoluchatel provided by mobile operator to accounting record of the portal;

2) the document which is subject to apostilization in the form of the scanned copy is attached to electronic request.

In case of change of biographical particulars of uslugopoluchatel, the supporting documents (issued or certified by competent organization of foreign state or specially on that the authorized person, within its competence and in the established form sealed official foreign state are in addition attached, procedures of the special certificate (legalization or apostilization) are adopted only after passing.

In case of the address of heirs, supporting documents are submitted.

Service providers receive digital documents from service of digital documents through the realized integration on condition of the consent of the owner of the document provided by means of the "electronic government" of subscriber number of cellular communication of the user registered on the web portal by transfer of the one-time password or departure siding of the short text message as the answer to the notification portal web of "the electronic government".

Other requirements taking into account features of rendering the state service including rendered electronically and through the State corporation

Uslugopoluchatelyam having the violation of health with permanent disorder of functions of organism limiting its life activity, in case of need documents acceptance for rendering the state service, is made by the worker the State corporation with departure at the place of residence by means of the address through Single contact center 1414, 8 800 080 7777.

Uslugopoluchatel has possibility of receipt of the state service electronically through the portal on condition of EDS availability.

Addresses of places of rendering the state service are placed on Internet resources of the service provider and the www.gov4c.kz State corporation.

Uslugopoluchatel obtains information on procedure and the status of rendering the state service in the mode of remote access by means of Single contact center 1414, 8 800 080 7777.

Service of digital documents is available to the users authorized in mobile application.

For use of the digital document it is necessary to undergo authorization in mobile application with use of the electronic and digital signature or one-time password, further to pass into the Section "Digital Documents" and to choose the necessary document.
