1.

Vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2 caused by coronavirus (further - COVID-19)

in the central database of electronic health care (further - the central database) in the Register of medical records, records about the direction and recipes there are data on full course of vaccination from COVID-19 (there are medical records about vaccination (inoculations, immunization) with indication of data on vaccine series which correspond to the information provided by the Ministry of Health on series of vaccines which are applied (in case of verification of data on series of vaccine availability of the blank places and special symbols, and also the distinctions connected with the register of letters are not considered and comparison of letters of the Ukrainian alphabet by Latin according to the table 2 "Comparison of the Cyrillic and Latin Letters Used when Forming COVID of Certificates" of this appendix is applied. In cases if the specified algorithms did not promote data transmission, necessary for forming of COVID certificate, search of series in the table of comparison of series of vaccines from COVID-19, which is updated by the administrator of the central database is applied. In cases if in the central database concerning one person there are several records about vaccination from COVID-19, in which numbering of doses of vaccination from COVID-19, for determination of completeness of rate of vaccination is incorrectly specified table 3 "The minimum and maximum terms between vaccination stages which will constitute complete cycle, for application in algorithm of check of compliance of the records about vaccination (inoculations, immunization) which are available in the central database confirming the fact of carrying out complete cycle of vaccination from COVID-19 "this appendix is used)

COVID certificate can be created right after entering into the central database of data concerning the last dose of vaccine according to the protocol of vaccination from COVID-19

Testing by method of polymerase chain reaction on COVID-19 (further - testing for COVID-19)

in the central database there are data on testing for COVID-19 with negative result;

COVID certificate can be created right after entering into the central database of data on receipt of negative result of testing for COVID-19

Recovery from COVID-19

in the central database there are data on the diagnosis of COVID-19, confirmed with positive result of testing for COVID-19 or rapid test for SARS-CoV-2 coronavirus antigen determination;

COVID certificate can be created for the fourteenth day after date of intake of biological material for testing for COVID-19, result of which positive, in the presence of data on recovery
