Changes which are made to the Procedure and conditions of appointment and payment of public welfare payments to the citizens having children, the Ministries of Labour and Social Protection of the Russian Federation approved by the Order of September 29, 2020 No. 668n

c) data on civil accessory of person having the right to benefits;

d) number of the policy of compulsory medical insurance of person having the right to benefits in case of the request for purpose of maternity allowance;

e) data on the professional educational organization, educational organization of the higher education, the organization of additional professional education or the scientific organization in which person having the right to benefits studies full-time;

e) the information about child(children) of person having the right to benefits (surname, name, middle name (in the presence), birth date, the Insurance Number of Individual Ledger Account);

g) the information about the adopted child(children) (surname, name, middle name (in the presence), birth date, the Insurance Number of Individual Ledger Account);

h) the information about the second parent in case of the request for purpose of lump-sum allowance in case of the child's birth (surname, name, middle name (in the presence), birth date, the Insurance Number of Individual Ledger Account);

i) the information about the child's father, in case of the request for purpose of lump-sum allowance to the pregnant wife of the serviceman undergoing military service and (or) monthly allowance on the child of the serviceman undergoing military service (surname, name, middle name (in the presence), birth date, the Insurance Number of Individual Ledger Account, data on military unit in which it undergoes military service, or the name of the military professional educational organization or military educational organization of the higher education, in which is trained, the name of military commissariat in the place of appeal (in case of the termination of military service by it);

j) contact information of person having the right to benefits;

k) information that person having the right to benefits is warned about responsibility for reliability of the provided data.".

5. In Item 8 of the word "in Item 6 of this Procedure" shall be replaced with words "in Item 7 of this Procedure".
