1. Approve the Procedure for provision to tax authorities of the Notice on opening (closing) of accounts in financial institutions (further the Procedure) which is applied.
2. Serdyuk N. P. to give to the chief of Head department of accounting and the state registers of taxpayers the specified Procedure to the Ministry of Justice of Ukraine for state registration.
3. Declare invalid the order of State Tax Administration of Ukraine of March 7, 1998 N100 "About Approval of Form and Content of the Notice on Opening (Closing) of Accounts of Subjects of Business Activity in Organizations of Banks" registered in Ministry of Justice on March 24, 1998 for N190/2630.
4. To provide to heads of State Tax Administrations in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol fulfillment of requirements of the Procedure.
5. To the head of department in ten-day time after state registration of the Procedure to provide with Kovalenko V. V. cases its replication and posylaniye to State Tax Administrations in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol.
