For the purpose of providing requirements of Item 2 of the resolution of the Cabinet of Ministers of Ukraine of July 5, 2006 N919 "About Approval of the Procedure for Wages Determination (Income) for Calculation of Pension in case of Loss of Documents on Its Charge and Payment" is ORDERED:
1. Approve the Procedure for obtaining from the State register of physical persons - taxpayers and other obligatory payments of data on the salary (income) for calculation of pension in case of loss of documents on its charge and payment (further - the Procedure) which is attached.
2. To provide to department of provision of pensions of the Ministry of Labour and Social Policy of Ukraine in accordance with the established procedure provision of this order on state registration in the Ministry of Justice of Ukraine.
3. Control over the implementation of this order is imposed on the deputy minister of work and social policy of Ukraine O. Goryachuyu, the vice-chairman of State Tax Administration of Ukraine L. Boyenko.
