1. Bring in the form of the declaration on property condition of the debtor on the case of insolvency approved by the order of the Ministry of Justice of Ukraine of August 21, 2019 No. 2627/5, registered in the Ministry of Justice of Ukraine on August 23, 2019 at No. 964/33935, the following changes:

1) in Item 1 of the Section I after the words "have mark in the passport)" to add with the words "unique number of entry in the Unified state demographic register";

2) in the Section VI:
