a) the statement addressed to the head of body or the organization of prosecutor's office with request for revenues to service;

b) passport of the citizen of the Russian Federation;

c) the documents confirming getting out of nationality (citizenship) of foreign state (loss of nationality (citizenship) of foreign state) or cancellation of the residence permit or other document confirming the right to permanent residence of the citizen of the Russian Federation in the territory of foreign state if the citizen had nationality (citizenship) of foreign state or the right to permanent residence in the territory of foreign state earlier;

d) questionnaire, stipulated in Item the 6th article 40.1 of this Federal Law;

e) education documents and about qualification with appendices;

e) documents on award of academic degree and on assignment of academic status (in the presence);
