1. Make changes to the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of the organization and carrying out gamblings approved by the decision of the Commission on regulation of gamblings and lotteries of October 25, 2021 No. 681, of Ukraine registered in the Ministry of Justice on November 15, 2021 at No. 1490/37112, having been reworded as follows it which is applied.

2. To provide to department of methodology together with department of legal providing in the procedure established by the legislation submission of this decision on state registration in the Ministry of Justice of Ukraine.
