date of opening of the special account;

date of the conclusion of bank account agreement (in the presence);

b) concerning physical person:

surname, name, middle name (in the presence);

birth date;

identification taxpayer number - the foreign agent;

number of the special account;

name and location of authorized bank;

bank identification code of authorized bank;

number of correspondent account (sub-account) of authorized bank (its branch);

date of opening of the special account;

date of the conclusion of bank account agreement (in the presence);

c) concerning foreign structures without formation of legal entity:
