1. In the resolution of the Cabinet of Ministers of Ukraine of April 25, 2018 No. 410 "About agreements on medical attendance of the population according to the program of medical guarantees" (The Official Bulletin of Ukraine, 2018, No. 45, Art. 1570; 2020, No. 2, Art. 59; 2021, No. 82, Art. 5250):
1) the paragraph the fifth Item 11 of the Procedure for the conclusion, change and termination of the contract on medical attendance of the population according to the program of medical guarantees approved by the specified resolution to state in the following edition:
"The subject of managing shall provide use by authorized persons and health workers which will be involved in the conclusion or agreement performance, the qualified digital signature or the advanced digital signature which is based on the qualified digital signature certificate (further - the digital signature). The qualified digital signature certificate in addition to identification data of physical person contains identification data of appropriate subject of managing.";
2) in the Standard form of the agreement on medical attendance of the population according to the program of medical guarantees approved by the specified resolution:
