About change of the resolution of the Ministry of Labour and Social Protection of the Republic of Belarus of August 25, 2016 No. 43
Based on part two of Item 9 of the Regulations on procedure and conditions of the passing by citizens of alternative service approved by the resolution of Council of Ministers of the Republic of Belarus of June 27, 2016 No. 497, and paragraph one of subitem 7.1 of Item 7 of the Regulations on the Ministry of Labour and Social Protection of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of October 31, 2001 No. 1589, the Ministry of Labour and Social Protection of the Republic of Belarus DECIDES:
1. Bring in the Instruction about procedure for filling of forms of the certificate, instruction, accounting card, their issue and accounting approved by the resolution of the Ministry of Labour and Social Protection of the Republic of Belarus of August 25, 2016 No. 43, the following changes:
in item 4 part six the word "record" shall be replaced with words "record or it is required to change record about surname, own name, middle name (if that is available), to birth date,";
add the Instruction with Item 7 of the following content:
"7. In case of change in accounting card of surname, own name, middle name (if that is available), the birth dates specified data are crossed out and over them the changed surname, own name, middle name (if that is available), and birth date registers. The specified changes are made on the first page of accounting card based on the identity document. The reference to the relevant document registers on inside of cover of accounting card, is certified by the signature of the authorized officer of the organization in which the citizen passes alternative service, with indication of initials, surname and is certified by seal of the organization, except for the subjects of managing having the right not to use seal according to legal acts.
