According to Items 12, 13 Lists of documents which are enclosed to the application for issue of licenses for the separate type of economic activity approved by the resolution of the Cabinet of Ministers of Ukraine of July 4, 2001 N 756, ORDER:
1. Approve:
1.1. Form of the certificate of condition of material and technical resources of business entity, availability at it of normative legal documents, including regulating documents on standardization, necessary for implementation of economic activity on medical practice to conversion of donor blood and its components, production of medicines from them (further - the Reference) which is applied.
2. Authorize the Ministry of Health of the Autonomous Republic of Crimea, management of health care of regional public administrations, Head department of health care and medical support Kiev and management of health care Sevastopol the city public administrations to issue based on statements of business entities the Reference according to the form determined by this order.
3. To the director of the department of inspection and quality control of medical services Shpak I. V. provide in accordance with the established procedure state registration of this order in the Ministry of Justice of Ukraine.
4. To impose control for accomplishment of the order on the deputy minister Musiyenko A. V.
