About approval of the Instruction about procedure for filling of registration cards of single state system of registration and accounting of offenses
In pursuance of article 8 of the Law of the Republic of Belarus of December 31, 2006 "About enforcement of the Code of the Republic of Belarus about administrative offenses and the Procedural and executive code of the Republic of Belarus about administrative offenses" the Ministry of Internal Affairs of the Republic of Belarus DECIDES:
1. Approve the enclosed Instruction about procedure for filling of registration cards of single state system of registration and accounting of offenses.
2. Declare invalid the resolution of the Ministry of Internal Affairs of the Republic of Belarus of August 18, 2006 No. 231 "About approval of the Instruction about procedure for filling of registration cards of single state system of registration and accounting of offenses" (The national register of legal acts of the Republic of Belarus, 2006, No. 149, 8/14963).

1. This Instruction determines procedure for filling with registering bodies, bodies of preliminary inquiry, the bodies performing execution of punishments, prosecutors, courts, accounting divisions of registration cards of single state system of registration and accounting of offenses (further registration cards).
2. In this Instruction the following main terms and their determinations are applied:
filling of registration card - fixing in registration card of data on offense, and also registration and accounting of offense;
registering body - state body or other state organization (its territorial structural subdivision or body), including court which workers constitute the protocol on administrative offense or the decree on imposing of administrative punishment according to parts one and third article 10.3 of the Procedural and executive code of the Republic of Belarus about administrative offenses is issued;
accounting division - information and analytical division of managements, Departments of Internal Affairs of city, district executive committees (local administrations), and also the departments subordinated to the Department of Internal Affairs on transport of the Ministry of Internal Affairs of the Republic of Belarus.
3. The reducings used in registration cards are given in the list of the reducings applied in registration cards of single state system of registration and accounting of offenses according to appendix 1.
4. Data on administrative offenses are fixed in the following registration cards:
about the administrative offense made by physical person (further - form 1-AP), according to appendix 2 *;

about the administrative offense made by the legal entity (further - form 3-AP), according to appendix 3;
about results of appeal (protest) of the resolution which took legal effect on the case of administrative offense (daleeforma 2-AP) according to appendix 4.
5. Data on crimes are fixed in the following registration cards:
about crime (further - form 1) according to appendix 5;
about the physical person which was injured as a result of crime (daleeform 1-P), according to appendix 6;
about the suspect, the person accused (further - form 1-L) according to appendix 7;
about the course of preliminary inquiry (further - form 2) according to appendix 8;
about the end of preliminary inquiry (further - form 3) according to appendix 9;
about person who committed crime (further - form 3-L), according to appendix 10;
about the decision of the prosecutor on criminal case (further - form 4) according to appendix 11;
about the course of consideration of criminal case in court (further - form 5) according to appendix 12;
about the person taken into custody, condemned (further - form 6) according to appendix 13;
about the location of the convict (further - form 7) according to appendix 14.
6. Registration cards consist of the Sections numbered by the Roman figures and the details numbered by the Arab figures. Names of Sections contain instructions on the group of people, performing their filling. In details homogeneous data on offenses, registration and accounting of offenses are fixed.
7. Forms of registration cards are made for the following forms:
1-AP, 2-AP, 3-AP, 1, 3, 3-L - on one sheet of paper of 210 x 297 mm in size with a density at least 80 g/m2;
6 - on one sheet of paper of 95 x 140 mm in size with a density at least 160 g/m2;
1-P, 1-L, 2, 4, 5, 7 - on one sheet of paper of 148х210 mm in size with a density at least 80 g/m2.
8. Forms of registration cards join details according to appendices 2-14 to this Instruction, except for the details intended for filling with accounting divisions directly in territorial databanks about offenses.
The fields of details intended for filling with figures are represented in the form of cages.
Samples of forms of registration cards are transferred on electronic channels to the interested state bodies within three working days after acceptance of the regulatory legal act changing (supplementing) content of their details.
