1. Approve form of permission to implementation of international carriages of radioactive materials and the application form of issue of the authority for the move of radioactive materials which are applied.
2. (Gilev O. G.) to provide to the central inspection management provision of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine and bringing to permission of the interested organizations of information on entry into force of this order.
3. In week time after state registration of the order to provide to information crisis management (Ananenko O. M.) its placement on Gosatomregulirovaniya's website.
4. To impose control of execution of the order on the vice-chairman - the Chief state inspector on nuclear and radiation safety of Ukraine Gashev M. H.
