1. Approve:
1) form of the Declaration on remaining balance and (or) turnover of tobacco products (further - the Declaration);
2) Rules of provision of the Declaration.
2. Determine that the Declaration is submitted monthly no later than the 15th following for reporting.
3. To management of administration of excises of the Tax Committee of the Ministry of Finance of the Republic of Kazakhstan (Kim R. Yu.) direct this order to state registration in the Ministry of Justice of the Republic of Kazakhstan.
4. This order becomes effective since January 1, 2004.
