About approval of report form about production volumes of medicines with use of ethyl alcohol

According to the paragraph 2 Items 8 of the Procedure for release, addresses and repayments of tax anticipation bills, avalized bank (tax receipts) which are issued before obtaining from the excise warehouse of the ethyl alcohol which is used for production of separate types of products, approved by the resolution of the Cabinet of Ministers of Ukraine of 23.08.2005 of N 796, and the Regulations on the Ministry of Health of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of 02.11.2006 of N 1542, ORDER:

1. Approve report form about production volumes of medicines with use of ethyl alcohol (further - the Report) which is applied.

2. The subjects of business activity receiving ethyl alcohol from excise warehouse for production of medicines (including components of blood and the medicines made of them) (further - Subjects), report in triplicate.

2.1. The report is approved by the representative of the State Tax Administration on tax post.

2.2. One copy of the Report moves in Department of regulatory policy in the field of drug circulation and products in MZ health care system (further - Department).

3. Provide to department:

3.1. Provision of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

3.2. Bringing contents of the order to permission of Subjects.

4. To impose control over the implementation of the order on the deputy minister Yurchenko V. D.

5. This order becomes effective from the date of official publication.
