About approval of the Nation-wide development program of primary health care on fundamentals of family medicine for the period till 2011

The Verkhovna Rada of Ukraine decides:

1. Approve the Nation-wide development program of primary health care on fundamentals of family medicine for the period till 2011 which is applied.

2. This Law becomes effective from the date of its publication.

General part

Deterioration in the state of health of the population, high rates of death rate of persons of working-age, reduction of average life expectancy, inequality in availability of medical care lead to objective increase in need for medical care which the existing health care system is not able to satisfy.

According to the recommendations of the World Health Organization prepared based on the best international experience, only development of primary health care on fundamentals of family medicine will give the chance to influence significantly improvement of demographic situation, to reach equitable distribution and rational use of budgetary funds.

In most the states of the world about 90 percent of total amount of medical services fall to the share of primary health care, and the specific weight of number of general practitioners - family doctors among doctors of field of health care makes 30-50 percent.

Primary health care is for today part of the specialized out-patient help therefore management of primary help using economic levers is impracticable.

For many years only 10 percent of the funds allocated from the government budget for health care that, in view of the low level of material logistics of noted institutions, is very insufficient were spent for content of the institutions of health care providing primary health care.

The principles of complexity and succession in provision of medical care are not applied. The sufficient attention is not paid to prevention of diseases and medical examination of the population. It leads to untimely detection of diseases and their complications and consequently, to increase in need of the population for specialized and highly specialized medical care.

 

Program purpose

The purpose of the Nation-wide development program of primary health care on fundamentals of family medicine for the period till 2011 (further - the Program) is ensuring decrease in the illness rate, disability and death rate of the population by forming and establishing effective functioning of system of provision to the population of available and high-quality primary health care on fundamentals of family medicine.

 

Ways and methods of the problem resolution

It is necessary for the problem resolution:

enhance normative and legal and scientific and methodical base, in particular concerning activities of institutions of health care which provide primary health care (daleelechebny institutions);

continue work on creation of network of out-patient clinics of family medicine in the city and rural zone by reorganization and reshaping of the operating institutions of health care, ensuring material equipping of such out-patient clinics;

carry out step-by-step restructuring of out-patient and polyclinic institutions for the purpose of creation of medical institutions;

provide availability of the population to primary health care;

increase level of training, retrainings and advanced trainings of doctors, other medical personnel for provision of primary health care;

enhance system of teaching bases of primary health care in the highest medical educational institutions and institutions of post-qualifying education;

carry out review of sheets of equipment of medical institutions with gradual providing them with the medical energy efficient equipment and specialized vehicles;

enhance procedure for licensing of economic activity on medical practice in "general practice - family medicine" and the state accreditation of institutions of health care;

develop and implement standards of primary health care and criteria for evaluation of its quality;

enhance system of the rating indicators characterizing quality of work of medical institutions, and procedure for their systematization;

promote development of international cooperation with the purpose of studying and distribution in Ukraine of the best international experience of the organization of primary health care on fundamentals of family medicine;

inform the public on feasibility of changes in health care system;

develop and implement the mechanism of participation of the public in management of health care system.

The predicted amounts of financing are given in appendix 1.

Tasks and actions for accomplishment of the Program are given in appendix 2.

 

The expected results, efficiency of accomplishment of the Program

Accomplishment of the Program will give the chance:

increase overall performance of institutions of health care for the purpose of overcoming unfortunate demographic trends;

increase specific weight of medical care which is provided:

general practitioners - family doctors, annually for 7-10 percent;

on fundamentals of family medicine at the level of the out-patient and polyclinic help, annually for 5-7 percent;

create system of provision to the population of high-quality medical care on fundamentals of family medicine;

create conditions for realization of the principle of the organization and coordination by the general practitioner - the family doctor of provision to patients of specialized and stationary medical care.

The expected results of accomplishment of the Program are given in appendix 3.

 

Amount and sources of financing of the Program

Ensuring accomplishment of this Program is performed in the expense limits provided in the Government budget of Ukraine and local budgets and also at the expense of means of other sources.

The amount of financing of the Program from the Government budget of Ukraine is determined annually proceeding from specific objectives and the available means and can be specified during creation of the draft of the Government budget of Ukraine for the corresponding year taking into account budget revenues opportunities.
