In the column "Documents Which Are Enclosed to the Application for Licensing for Separate Type of Economic Activity" of Item 44 of the list of documents which are enclosed to the application for licensing for the separate type of economic activity approved by the resolution of the Cabinet of Ministers of Ukraine of July 4, 2001 N 756 (The Official Bulletin of Ukraine, 2001, N 27, by Art. 1212; 2005, N 22, to the Art. 1199) to state paragraph two in the following edition:

"for economic activity on wholesale, retail trade by medicines - the passport copy of pharmaceutical institution (structural division), constituted in the form approved by the Cabinet of Ministers of Ukraine, certified by the signature of the head of the subject of housekeeping, under seal the subject of housekeeping, on each page".
