1. Approve blank forms of civil registrations: the birth, death, marriage, annulment of marriage, adoption (adoption), paternity proof, change of surname, name and middle name, recovery of the birth statement, blank form of other documents confirming the facts of state registration of acts of civil status and blank form of statements for state registration of acts of civil status (appendix 1-71).

2. Enact the following blank forms of the strict reporting:

- certificates of birth and adoption (adoption) - since January 1, 2011;

- certificates on marriage and death - since February 1, 2011;

- certificates on annulment of marriage and change of surname, name and middle name - since March 1, 2011;

- affiliation certificates - since January 1, 2012.

3. Establish the cost of 1 unit of record form of the act of civil status in the amount of 20 percent from indicator for calculations with money transfer into the special account of the Ministry of Justice of the Republic of Tajikistan.
