1. Approve the enclosed Procedure for representation and control of the reporting on education, use, neutralization and placement of waste (except for the statistical reporting).

2. Determine that the reporting on education, use, neutralization and placement of waste (except for the statistical reporting) for 2009 is submitted in the relevant territorial authorities of Federal Service for Environmental, Technological and Nuclear Supervision till July 1, 2010.

Procedure for representation and control of the reporting on education, use, neutralization and placement of waste (except for statistical reporting)

1. The procedure for representation and control of the reporting on education, use, neutralization and placement of waste (except for the statistical reporting) (further - the Procedure) establishes requirements to content and submission of the reporting on education, use, neutralization, about placement of waste (further - the Reporting).

2. The procedure is intended for legal entities and individual entrepreneurs as a result of whose economic and other activity waste and which according to the Federal Law of July 24, 2007 N 209-FZ "About development of small and medium entrepreneurship in the Russian Federation" (The Russian Federation Code, 2007, N 31, of the Art. 4006, N 43, of Art. 5084 are formed; 2008, N 30, Art. 3615, Art. 3616; 2009, N 31, the Art. 3923, N 52, Art. 6441) concern to subjects of small and medium entrepreneurship, and for territorial authorities of Rosprirodnadzor.

3. Subjects of small and medium entrepreneurship represent in notifying procedure the Reporting to territorial authorities of Rosprirodnadzor in the place of implementation of the economic and other activity as a result of which waste are formed.

4. The accounting period constitutes one calendar year.

In case of the beginning of implementation of economic and other activity by the subject of small and medium entrepreneurship within accounting calendar year the accounting period is estimated from the date of state registration of the legal entity or individual predprinimatelyasubjekt of small and medium entrepreneurship.

5. The reporting is submitted till January 15 of the year following the accounting period.

The reporting is constituted on paper in duplicate, one of which is stored at the reporting subject of small and medium entrepreneurship, and the second, together with the electronic version of the Reporting on the magnetic carrier, - Rosprirodnadzor is represented in the relevant territorial authority.

7. The reporting is constituted on the basis of the data of primary accounting formed, used, neutralized, transferred to other legal entities and individual entrepreneurs, received from other legal entities and individual entrepreneurs or physical persons, and also the placed waste.

8. The reporting includes:

1) general information about the reporting subject of small and medium entrepreneurship;

3) the information about legal entities and individual entrepreneurs to whom waste was transferred in the accounting period;

b) for individual entrepreneurs:

surname, name and middle name (the last - in the presence), the residence, data of the identity document, the state registration number of record about state registration as the individual entrepreneur and data of the document confirming the fact of entering of record about the individual entrepreneur into the Unified State Register of Private Entrepreneurs;

phone, fax, e-mail;

code of the residence on the All-Russian qualifier of subjects to administrative-territorial division (RCATU).

10. The balance of the masses formed, used, neutralized, transferred to other legal entities and individual entrepreneurs, received from other legal entities and individual entrepreneurs or physical persons, the placed waste for the accounting period includes data on masses (in tons) formed, used, neutralized, transferred to other legal entities and individual entrepreneurs, received from other legal entities and individual entrepreneurs or physical persons, placed on own subjects to storage/waste disposal, cumulative (the last day of the accounting period) the waste grouped in each type of waste with indication of its name, code according to the federal classification catalog of waste, danger class.

the locations or residences and their codes on the All-Russian qualifier of subjects to administrative-territorial division (RCATU);

date of issue and the document number confirming availability of the license for activities for collection, use, neutralization, transportation, placement of waste of the I-IV class of danger (by transfer of waste of the I-IV class of danger);

dates and agreement numbers about transfer of waste;

data on masses (in tons) the transferred waste grouped in each type of waste with indication of its name, code according to the federal classification catalog of waste, class of danger, the transfer purpose (use, neutralization, placement).

12. Appendices to the Reporting include:

1) the copy of the document confirming availability of the license for activities for collection, use, neutralization, transportation, placement of waste of the I-IV class of danger - in case of implementation by the reporting subject of small and medium entrepreneurship of activities for collection, use, neutralization, transportation to placement of waste of the I-IV class of danger. The copy of the document confirming license availability is certified by seal of the reporting subject of small and medium entrepreneurship;

2) copies of contracts for transfer of waste to other legal entities and individual entrepreneurs or on acceptance of waste from other legal entities and individual entrepreneurs for the accounting period and copies of delivery-acceptance certificates of waste under the specified agreements. Copies of the specified agreements and acts are certified by seal of the reporting subject of small and medium entrepreneurship;

3) copies of acceptance acts of acceptance of scrap and waste of ferrous and non-ferrous metals;

4) copies of the documents confirming availability of the license for activities for collection, use, neutralization, transportation, placement of waste of the I-IV class of danger and issued to legal entities and individual entrepreneurs to whom the reporting subject of small and medium entrepreneurship transferred in the accounting period waste of the I-IV class of danger. 16. Rosprirodnadzor posts on the official site on the Internet for open and free entry the electronic version of execution of the Reporting and the software for its forming.
