About approval of models of assembly records of civil status, descriptions and samples of forms of certificates on state registration of acts of civil status

According to part three of Article 9 and part one of article 18 of the Law of Ukraine "About state registration of acts of civil status" the Cabinet of Ministers of Ukraine decides:

1. Approve enclosed below:

models of assembly records of civil status;

descriptions and samples of forms of certificates on state registration of acts of civil status.

2. Determine that:

available remaining balance of forms of certificates on state registration of acts of civil status which samples are approved by the resolution of the Cabinet of Ministers of Ukraine of September 12, 2002 N 1367 "About approval of samples of books of civil registration and descriptions of forms of certificates which are granted by state bodies of civil registration" (The Official Bulletin of Ukraine, 2002, N 38, the Art. 1775), is used by bodies of state registration of acts of civil status in full;

the certificates on state registration of acts of civil status published to the introduction in operation of this resolution are valid and are not subject to exchange.

3. Determine series of forms of certificates on state registration of acts of civil status which are granted by bodies of state registration of acts of civil status, according to appendix.

4. Declare invalid:

the resolution of the Cabinet of Ministers of Ukraine of September 12, 2002 N 1367 "About approval of samples of books of civil registration and descriptions of forms of certificates which are granted by state bodies of civil registration" (The Official Bulletin of Ukraine, 2002, N 38, the Art. 1775);

the resolution of the Cabinet of Ministers of Ukraine of March 12, 2003 N 316 "About introduction of amendments to the resolution of the Cabinet of Ministers of Ukraine of September 12, 2002 N 1367" (The Official Bulletin of Ukraine, 2003, N 11, the Art. 484);
