For the purpose of ensuring practical application of provisions of subitem 169.2.4 of Item 169.2 of Article 169 of the Section IV of the Tax code of Ukraine, being guided by article 8 of the Law of Ukraine "About the State Tax Service in Ukraine", I ORDER:

1. Approve the Procedure for informing employers of the taxpayer on availability of violations of application of tax social benefit, deprivation of the taxpayer or about renewal of its right to tax social benefit it (is applied).

2. Approve form of the Message on availability of violations of application of tax social benefit, deprivation of the taxpayer or of renewal of its right to tax social benefit it (is applied).

3. Declare invalid the order of State Tax Administration of Ukraine of 30.09.2003 N 461 "About approval of forms of statements and notices concerning receipt (application) of tax social benefit, and also procedure for informing taxpayers", registered in the Ministry of Justice of Ukraine 14.10.2003 for N 929/8250.

4. To department of the taxation of physical persons (Dusyak S. P.):

4.1 in accordance with the established procedure to submit this order on state registration to the Ministry of Justice of Ukraine;

4.2 provide publication of the order in accordance with the established procedure.

5. This order becomes effective from the date of official publication.
