About approval of form of the Tax declaration on income tax

1. Approve form of the Tax declaration on income tax (further - the declaration) which is applied.

2. To department of the taxation of physical persons (Dusyak S. P.) in accordance with the established procedure:

2.1 submit this order to the Ministry of Justice of Ukraine for state registration;

2.2 provide promulgation of this order;

2.3 provide in five-day time of day of registration of the order in the Ministry of Justice of Ukraine provision to Department of information and analytical ensuring processes of the taxation (Laba M.S.) control algorithms for acceptance and the automated processing of the declaration.

3. To department of information and analytical ensuring processes of the taxation (Laba M.S.), to Management of registration and accounting of taxpayers (Kalenichenko N. G.) to finish the software on acceptance and processing of tax statements of taxpayers regarding data processing of the declaration according to the procedure and the terms determined for creation of information systems.

4. This order becomes effective from the date of official publication.

5. To impose control over the implementation of the order on the acting as the First Deputy Chairman Lekar of S. I.
