About modification of the Regulations on procedure for investigation and accounting of violations in work of nuclear power plants

According to article 24 of the Law of Ukraine "About use of nuclear energy and radiation safety" and for the purpose of ensuring state regulation in the field of use of nuclear energy I ORDER:

1. In the title and Item 1 of the order of Gosatomregulirovaniya of 01.12.2004 No. 184 "About approval of the Regulations on procedure for investigation and accounting of violations in work of nuclear power plants", registered in the Ministry of Justice of Ukraine 17.12.2004 for No. 1594/10193, the word "electric" to exclude.

2. Make changes to the Regulations on procedure for investigation and accounting of violations in work of nuclear power plants approved by the order of the State committee of nuclear regulation of Ukraine of 01.12.2004 No. 184, registered 17.12.2004 for No. 1594/10193, having been reworded as follows it which is applied the Ministry of Justice of Ukraine.

3. To department of assessment of safety of nuclear installations (Shevchenko I. A.) provide submission of this order in five-day time on state registration in the Ministry of Justice of Ukraine.

4. To department of assessment of safety of nuclear installations (Shevchenko I. A.) provide mailing of this order according to calculation of mailing after its replication.

5. This order becomes effective from 01.01.2012.

6. To impose control over the implementation of this order on the First Deputy Chairman - the Chief state inspector on nuclear and radiation safety of Ukraine Gashev M. H.

Regulations on procedure for investigation and accounting of violations in work of nuclear power plants

I. Terms and determinations

In this Provision terms are used in such value:

accident - breakdown of service of nuclear power plant (further - the EXPERT) through which there was exit of radioactive materials and/or ionizing radiation for the borders provided by the project for normal operation in quantity which exceeds the established borders of safe operation. Accident is characterized by initial incident, ways of course and effects;

accident nuclear - the accident connected with damage of the fuel element (дальшеТВЭЛ) which exceeds the established borders of safe operation, caused by yadernofizichesky processes owing to:

violations of control and management of chain reaction of separation in active zone;

formations of critical mass during overload, transportations and storages of fuel elements;

violations of the heat sink from fuel elements;

similar violations - violations in work the EXPERT which have at least one abnormal incident with the same immediate and root causes;

abnormal incident - variation from the normal mode of operation which can be caused by equipment failure, external influence, human error or shortcomings of the procedure;

nuclear power plant - the production and technological complex designed for energy production with use of nuclear installation (installations), located in borders of the territory determined by the project and staffed by necessary personnel;

immediate cause of abnormal incident - defect, action, divergence or condition which led to emergence of abnormal incident;

commissioning - process during which systems and elements of the power unit EXPERT or EXPERT in general begin to function and be checked for compliance to their project. Process includes precommissioning adjustment work, physical and energy start-up, trial operation and the EXPERT in commercial operation comes to the end with acceptance;

case - violation of normal operation the EXPERT or emergency;

equipment failure - incident which consists in violation of operating state of the equipment;

renewal - transfer process of object in operating state from disabled;

variation - the abnormal incident on the EXPERT which does not have effects which are given in appendix 1 to this Provision;

defect - each separate discrepancy of object to the established requirements;

additional shortcoming - undesirable incident (condition) which was revealed during investigation of violation in work the EXPERT, but did not influence its origin and development;

operation - all activities which are directed to achievement by safe method of the purpose for which the EXPERT, including work on capacities, start-up, stops, testing, maintenance, repair, overload of nuclear fuel, inspection was constructed during operation and another, the activities connected with it;

removal from operation - complex of actions after withdrawal of nuclear fuel and the termination of operation of installation which makes impossible its use for the purpose of, for which it was constructed, and ensures safety of personnel, the population and the environment;

the adjusting action - operation which is performed in response to emergence of violation in work the EXPERT and directed to elimination of effects of violation, prevention of repeating of similar violation in the future and/or on increase in safe operation of the power unit;

root cause of abnormal incident - circumstance which entailed conditions for identification of immediate cause;

not working capacity - condition of object in case of which it is not capable to carry out at least one of functions which accomplishment is required by operational, normative and technical and/or design (project) documentation;

non-operational condition - condition of object in case of which he does not answer at least one of requirements operational, normative мехнической and/or design (project) documentation;

human error - single unintentional wrong action during control of the equipment, the single omission of the correct action or single unintentional wrong action in maintenance time and the repair of the equipment and systems important for safety;

the wrong decision - the wrong unintentional accomplishment or failure to carry out of number of consecutive actions because of the wrong assessment of the current processes;

violation in work the EXPERT - case or accident which are characterized by categories A01-A04, P01-P 10, given in appendix 1 to this Provision;

violation technological - abnormal incident on the EXPERT which is caused by equipment failure the EXPERT of the 4th class of safety (turbine plants and the equipment of the main electric circuit the EXPERT with their auxiliary systems) and which the EXPERT has no the signs and effects established by this Provision for violations and variations in work is classified by categories "refusal of the I category" or "refusal of the II category";

working capacity - condition of object in case of which values of all parameters which characterize capability to perform necessary functions, meet the requirements of operational, normative and technical and/or design (project) documentation;

the operation termination - the final stage of operation of installation which is implemented after decision making about its removal from operation and during which it is given to condition when nuclear fuel is absent in its territory or, being in limits of this territory, it is placed only in the storages of the fulfilled nuclear fuel intended for long-term safe storage;

damage - incident which consists in violation of serviceable condition of object when its working capacity remains;

repair - complex of transactions for renewal of serviceable condition or operability of object and renewal of resources of object or its components;

serviceable condition - object condition which is characterized by its full compliance to all requirements of operational, normative and technical and/or design (project) documentation;

the concurrent factor - the reason which elimination will not allow to avoid repeating of abnormal incident, but which owing to the importance (that is influences on origin or strengthening of abnormal incident) needs development of the adjusting actions;

maintenance - complex of transactions or transaction for support of serviceable condition or operability of object in case of its use to destination, in idle time, storages and transportations.

II. General provisions

2.1. This Provision the EXPERT, approvals of the adjusting actions for their elimination and realization of these actions establishes categories, accounting treatment, transmissions of messages and investigations of violations, determination and elimination of the reasons of violations in work.

2.2. Requirements of this Provision are mandatory all legal entities and physical persons which perform the activities connected with commissioning, operation, removal from operation of power units EXPERTS.

2.3. In this Provision such reducings are used:

AZ - emergency protection;

VVER - the water-to-water power reactor;

GTsN - the main circulation pulser;

PEL - the absorbing element;

OUSE - the accelerated protection;

RUM - the capacity limitation relay;

RU - reactor installation;

SVB - the systems important for safety;

SUSA - management system and protection;

TVS - the heat-generating collection;

TRBE - production schedules of safe operation;

BRU-SP - high-speed reducing installation with dumping of vapor on own requirements;

BRU-D - high-speed reducing installation with dumping of vapor into the deaerator;

BRU-K - high-speed reducing installation with dumping of vapor into the condenser.
