About modification and amendments in the resolution of Council of Ministers of the Republic of Belarus of July 8, 1997 No. 837

For the purpose of enhancement of conducting the individual (personified) accounting of insured persons in system of the national social insurance the Council of Ministers of the Republic of Belarus DECIDES:

1. Bring in Rules of the individual (personified) accounting of insured persons in system of the national social insurance, the Republic of Belarus approved by the resolution of Council of Ministers of July 8, 1997 No. 837 (The national register of legal acts of the Republic of Belarus, 2009, No. 83, 5/29527), the following changes and amendments:

1.1. in part two of Item 3 of the word "provision of pensions and professional pension insurance" shall be replaced with words "professional pension insurance, provision of pensions";

1.2. in part three of Item 7 of the word "of January 10, 2000 " about the electronic document " (the national register of legal acts of the Republic of Belarus, 2000, No. 7, 2/132)" shall be replaced with words "of December 28, 2009 "About the electronic document and the digital signature" (The national register of legal acts of the Republic of Belarus, 2010, No. 15, 2/1665)";

1.3. state Item 9 in the following edition:

"9. The PU-2 form is filled and it is represented:

on the insured persons working according to the employment contract – the employer based on orders (orders) on employment and dismissal from work;

on the insured faces consisting (consisting) in employment relationships – the employer with whom are drawn up stops) employment relationships;

on the insured persons which are temporarily transferred to work to other employer in connection with business necessity or in case of idle time – this employer;

in case of resubordination, reorganization of the organization and change of the owner of property – the employer with whom employment relationships of insured person proceed.";

1.4. in Item 10:

in word part one "special years of service" shall be replaced with words "the period (periods) of employment of workers at the works which are subject to inclusion in special years of service";

add Item with part three of the following content:

"On the physical persons who are independently paying compulsory insurance premiums on condition of absence of debt on their payment the PU-3 form is filled and it is represented the employee of body of Social Security Fund of the population based on data on the paid compulsory insurance premiums and is signed by its digital signature.";

1.5. in Item 12 of the word "professional years of service" shall be replaced with words "the period (periods) of employment of workers in special working conditions and separate types of professional activity (further – special working conditions)";

1.6. in part two of Item 15 of the word "10 days" shall be replaced with words "5 working days";

1.7. state Item 19 in the following edition:

"19. The PU-6 form (form type – initial) is represented in body of Social Security Fund of the population the insurer once a quarter along with the state statistical reporting under form 4 payments (Ministry of Labor and social security) "Payment report on professional pension insurance in Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus", the Republic of Belarus approved by the resolution of National Statistical Committee of May 14, 2011 No. 84, and contains the data relating to the accounting period.

The PU-6 form (types of forms – canceling and adjusting) is represented in case of need.

The PU-6 form (form type – award of pension) is represented in cases:

document creations for purpose (recalculation) of work retirement pension for work with special working conditions, work pension for long service, professional pension;

achievements of generally established retirement age by insured person;

dismissals of the worker who in current period reaches generally established retirement age.

In case of document creation for purpose (recalculation) of work retirement pension for work with special working conditions, work pension for long service, professional pension the PU-6 form (form type – award of pension) is represented within 5 working days from the date of submission of the corresponding statement by insured person to the employer, in body for work, employment and social protection, in body of Social Security Fund of the population.

In cases, the specified in paragraphs three and the fourth parts three of this Item, the PU-6 form (form type – award of pension) it is represented no later than the month following after month of achievement by insured person of generally established retirement age or dismissal of the worker which in current period reaches generally established retirement age.

If during the accounting period on insured person the PU-6 form was provided (form type – award of pension) and insured person continues to work for this insurer, in the month following reporting quarter the PU-6 form is represented (form type – adjusting).";

1.8. add Item 20 with part two of the following content:

"The payers of insurance premiums who are not undergoing state registration according to the Decree of the President of the Republic of Belarus of January 16, 2009 No. 1, represent the PU-6 PU-2, PU-3, forms no later than day of giving to body of Social Security Fund of the population of the termination letter of the activities (liquidation, reorganization, closing of representation and (or) branch), the termination of private notarial activities, about the expiration of action of employment and (or) civil contracts, refusal to participate in legal relationship on the national social insurance.";

1.9. to it to exclude the column "The Relations Regulated by the Labor Code of the Republic of Belarus (Code) *" and the interlinear note from appendix 2 to these Rules;

1.10. appendix 3 to these Rules to be reworded as follows (is applied);
