About approval of form of the Certificate of the submitted declaration on property condition and the income (tax declaration)

According to Item 179.12 of Article 179 of the Section IV of the Tax code of Ukraine I ORDER:

1. Approve form of the Certificate of the submitted declaration on property condition and the income (tax declaration) which is applied.

2. To the state tax authorities in areas, the cities (except the cities of Kiev and Sevastopol), areas in the cities, to the interdistrict, joint and specialized state tax authorities to provide issue to taxpayers of Certificates of the submitted declaration on property condition and the income (tax declaration) within three working days from the date of receipt of the statement from the taxpayer.

3. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.) and to Department of the taxation of physical persons of the State Tax Administration of Ukraine (Busarev V. V.) in the procedure established by the legislation:

submit this order on state registration to the Ministry of Justice of Ukraine;

provide promulgation of this order.

4. This order becomes effective since January 1, 2012, but not earlier than day of official publication.

5. To impose control over the implementation of this order on the First Deputy Minister of Finance of Ukraine Myarkovsky A. I. and vice-chairman of the State Tax Administration of Ukraine Ignatov A. P.
