According to provisions of Item 46.5 of Article 46 of the Section ІІ the Tax code of Ukraine I ORDER:

1. Approve form of the Tax declaration on the income tax of bank (the daleenalogovy declaration) which is attached.

2. Declare invalid the order of State Tax Administration of Ukraine of 21.02.2011 No. 98 "About approval of form of the Tax declaration on the income tax of bank", registered in the Ministry of Justice of Ukraine 17.03.2011 for No. 359/19097.

3. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Lyubchenko O. M.) in accordance with the established procedure to provide:

provision of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. To the State Tax Service of Ukraine (Klimenko O. V.):

in the three-months time of cells of day of state registration of this order in the Ministry of Justice of Ukraine to provide development of algorithms of control of acceptance and the automated processing of the Tax declaration;

finish the software on acceptance and processing of tax statements of taxpayers regarding data processing of the Tax declaration according to the procedure and the terms determined for creation of information systems.
