2) form of the decision on refusal in statement on registration accounting on the value added tax;

3) form of the decision on removal from registration accounting on the value added tax;

4) form of the decision on removal from registration accounting of the taxpayer performing separate types of activity;

5) form of registration card of cash register;

6) form of permission of tax authority to violation of integrity of seal of cash register.

2. This resolution becomes effective since January 1, 2012 and is subject to official publication.
