1. Approve the following forms for monitoring and assessment of narcological situation in criminal executive system of the Republic of Kazakhstan:
1) information on condition of narcological incidence in organizations of Committee of criminal executive system of the Ministry of Internal Affairs of the Republic of Kazakhstan, according to appendix 1 to this order;
2) the report on the contingents of patients in correctional facilities with dependence on psychoactive agents and associated diseases, according to appendix 2 to this order.
2. Two times a year (on July 15 and on January 15) to provide to committee of criminal executive system of the Ministry of Internal Affairs of the Republic of Kazakhstan in the Ministry of Health of the Republic of Kazakhstan information on condition of narcological incidence in organizations of Committee of criminal executive system of the Ministry of Internal Affairs of the Republic of Kazakhstan according to the forms approved by this order.
3. To the Ministry of Health of the Republic of Kazakhstan to carry out monitoring and assessment of narcological situation in criminal executive system of the Republic of Kazakhstan with provision of the state-of-the-art review to Committee of criminal executive system of the Ministry of Internal Affairs of the Republic of Kazakhstan.
