About approval of forms of sanitary and epidemiologic monitoring

1. Approve the enclosed forms of sanitary and epidemiologic monitoring according to appendix.
2. To heads of departments (managements) of the state sanitary and epidemiological surveillance of the cities of Astana, Almaty, areas, on transport:
To implement 1) since June 19, 2006 sanitary and epidemiologic monitoring in the forms approved by this order;
Reports of the quarterly and accruing sanitary and epidemiologic monitoring to represent 2) quarterly to the 20th of the month following reporting quarter to Committee.
3. Repeal orders of the Chief state health officer of the Republic of Kazakhstan of June 24, 2003 No. 39 "About the state sanitary and epidemiological surveillance behind economic and drinking water supply", of May 28, 2004 No. 17 "About introduction of monitoring of quality of water of open and underground water sources, centralized and not centralized water supply" and of July 7, 2004 No. 22 "About introduction of amendments to the order of the Chief state health officer of the Republic of Kazakhstan of May 28, 2004 No. 17".
3. To impose control of execution of this order on the vice-chairman of Committee Main thing of the state health officer of the Republic of Kazakhstan Askarov A. M.
