1. Approve Regulations on the list and requirements concerning form and content of documents which are filed for receipt of the license for carrying out separate types of activity in the field of use of nuclear energy which is applied.

2. To provide to legal department (Matveev V. G.) submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

3. This order becomes effective since October 1, 2012.

4. I reserve control over the implementation of this order.
