According to Item 179.3 of Article 179 of the Section IV of the Tax code of Ukraine and the subitem 64 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the Presidential decree of Ukraine of April 08, 2011 No. 446, I ORDER:

1. Approve form of the Certificate of the tax discharge on the income of physical persons as the taxpayer - the resident who goes abroad on the permanent place of residence, and about lack of the tax liabilities on this tax which is applied.

2. Declare invalid the order of State Tax Administration of Ukraine of December 22, 2010 No. 981 "About approval of form of the Certificate of the tax discharge on the income of physical persons by the taxpayer - the resident who goes abroad on the permanent place of residence, and about lack of the tax liabilities on this tax", registered in the Ministry of Justice of Ukraine on December 30, 2010 for No. 1422/18717.

3. (Chmeruk M. O.) and to Department of the taxation of physical persons of the State Tax Administration of Ukraine (Busarev V. V.) in the procedure established by the legislation to provide to department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

4. This order becomes effective from the date of its official publication.
