According to Item 46.5 of Article 46 of Chapter 2 of the Section II, Item 152.4 of Article 152 of the Section ІІІ the Tax code of Ukraine and the subitem 64 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the Presidential decree of Ukraine of April 08, 2011 No. 446, I ORDER:

1. Approve form of Calculation of the tax liabilities concerning payment of the consolidated income tax (further - Calculation) and form of the Notice of the taxpayer of decision making concerning payment of the consolidated income tax which are applied.

2. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Pavelko L. G.) to provide in accordance with the established procedure:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

3. To the State Tax Service of Ukraine:

in three-day time of day of state registration of this order in the Ministry of Justice of Ukraine to provide development of algorithms of control of acceptance and the automated Calculation processing;

finish the software on acceptance and processing of tax statements of taxpayers regarding Calculation data processing according to the procedure and the terms determined for creation of information systems.
